چکیده

A 43-year-old man from a rural area in Brazil presented to his physician with complaints of palpitations and lightheadedness, mainly on exertion. An electrocardiogram (ECG) showed right bundle-branch block (RBBB), left anterior fascicular block (LAFB), and a premature ventricular contraction (PVC) (Figure 1A). Echocardiography revealed a mildly depressed left ventricular (LV) ejection fraction (EF) of 46%, a slightly increased LV end-diastolic diameter (60 mm), and a LV apical aneurysm (Figure 1B and 1C). Subsequent 24-hour ambulatory ECG monitoring (Holter) recorded 4,055 isolated polymorphic PVCs, 236 couplets, and 17 short runs of nonsustained Editor: Albert I. We report the case of 52-year-old female with recurrent episodes of palpitations and dizziness. Holter monitoring revealed transient ST elevations followed by episodes of polymorphic ventricular tachycardia associated with episodes of palpitations and dizziness. Coronary angiography revealed mildly irregular right coronary artery with 90% stenosis. The patient underwent percutaneous coronary in...

Mid-Ventricular Hypertrophic Obstructive Cardiomyopathy Complicated by an Apical Aneurysm, Presenting as Ventricular Tachycardia

A 69-year-old woman presented to the emergency department with palpitations and dizziness of half an hour duration. The patient’s history included dyslipidemia under atorvastatin. The patient was hemodynamically stable. The 12-lead surface electrocardiogram demonstrated sustained ventricular tachycardia (VT) with a left ventricular origin and north-west axis at 200 beats/min (Fig. 1A). Cardiove...

Ventricular Tachycardia Induced by Weight Loss Pills

A previously healthy 29-year-old man was admitted with palpitations, dizziness, and near-syncope after he had recently started taking weight loss pills purchased on the internet. The pills contained caffeine and ephedrine. An electrocardiogram and telemetry revealed multiple episodes of non-sustained monomorphic ventricular tachycardia, which was successfully treated with amiodarone. In conclus...

Pressure Tracings in Hypertrophic Cardiomyopathy With Dynamic Midventricular Obstruction

A 50-year-old man was admitted for evaluation of syncope. Clinically, he had an irregular pulse and an ejection systolic murmur loudest at the left sternal border. Transthoracic echocardiography revealed hypertrophic cardiomyopathy (HCM) with midventricular obstruction and apical aneurysm with maximum pressure gradient greater than 74 mm Hg. Coronary angiogram showed normal coronary arteries. L...

It is associated with arrhythmias, heart failure and sudden death.1 Typical symptoms include palpitations, dizziness and syncope. Sudden death is often the first sign, especially in young people, and it coincides with so...
